  • What do I need to bring to my appointment?

    Please bring your insurance card, physician referral forms (if required by insurance) and a list of current medications with dose and frequency. Also, bring any MRI/CT/X-rays that you have had since your previous visit. Make sure to bring the actual CD containing the images NOT just the report. Please fill out the patient forms prior to your appointment.

  • What sites are best to research problems?

    Visit the links section for some specific recommendations. We do not endorse all of the information contained within these sites but they serve as a good starting point. In general, medical information presented by a major society or association (such as the American Academy of Neurology aka AAN) or an online journal assembled by physicians from peer-reviewed journals (such as eMedicine.com) are typically both accurate and up-to-date. The AAN puts out an electronic bimonthly publication for patients and their families entitled Neurology Now that is an excellent resource.

  • Which insurance do you accept and how is billing handled?

    Neurology Consultants of Montgomery is a provider for Medicare and most major insurance plans. We provide insurance billing. Anything not covered by insurance will be your responsibility.

    We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan.

    If you have any questions regarding which insurance plans we accept or any patient billing concerns, please call us at the phone number below. Questions regarding your coverage and benefits should be directed to your employer or insurance company.

  • What is the practice privacy policy?

  • What if I see another neurologist?

    Please have them fax a copy of their report to us so that we may coordinate your care in the best possible way. We will use our discretion as to whether to continue seeing you in a joint fashion.
